If Retaining an Attorney for Assiatance With A Private Adoption:
  1. Copy of original birth certificate if available
  2. Name, addresses, birth dates of parents and child to be adopted

If Retaining an Attorney for a Step-Parent Adoption or Adoption through CYF, bring:
  1. All of the information required for a private adoption;
  2. Documentation regarding termination of parental rights for biological parent(s)
  3. All court orders and documents relating to the child to be adopted

If Retaining an Attorney for Representation of a Birth Parent, bring:
  1. Name, address and medical information of birth parent
  2. Name of doctor and hospital where child will be delivered
  3. Name, address and birth date(s) of the adoptive parent(s)
